You could see the clicks. You could see the leads. You couldn’t see who walked in.

Every ad dashboard was green — impressions, clicks, cost-per-lead all trending up. None of it answered the only question a clinic cares about: did a patient actually come through the door? That blind spot is where the money was quietly bleeding.

How we did it

Three moves. In order.

We built one dashboard both teams could see.
Before touching a single campaign, we built a report around the number that actually matters — walk-ins — and put the performance team and the clinic’s sales team on it together. Leads → appointments → walk-ins, one shared view. Once everyone optimised for the same number, everything else followed. That was the number-one thing we did.

We saw Meta’s cost per walk-in, and killed it.
With walk-ins finally visible, Meta’s real cost showed up: ₹47,546 per walk-in in April, versus ₹1,698 on Google — 28× worse, one patient from ₹47.5K of spend. We didn’t nurse it. We cut it and moved the budget to what was booking patients.

We raised Target-CPA and tightened the copy.
A higher Target-CPA let Google bid straight at a cost-per-result, and sharper ad copy pulled in higher-intent patients — people ready to book, not just browse. Cheaper walk-ins, and more of them.
